The Best Way to Sleep When You Have Back Pain

Pain and sleep don’t mix well. Whether it’s a stiff neck, a sore lower back, or nerve pain down your leg, getting a good night’s rest can feel nearly impossible when you’re hurting.

Why Pain Often Feels Worse at Night

When you’re lying still for hours, your spine can stiffen. Inflammation can also pool in irritated joints or nerves while you rest. Gravity is no longer helping to pull excess fluid away, and your muscles aren’t moving to pump blood and lymph through the area.

If you already have:

  • A disc injury
  • Sciatica or pinched nerves
  • Neck or low back pain
  • Shoulder or hip strain

…your sleeping position matters even more.

Best Sleeping Positions When You’re in Pain

1. Side-Lying with a Pillow Between Your Knees

Best for: Lower back pain, sciatica, hip discomfort

This position helps keep your pelvis level, relieves tension on the lower spine, and reduces twisting through the lumbar joints.

Tips:

  • Sleep with your knees slightly bent (not curled tightly)
  • Use a firm pillow between your knees
  • Try placing a small pillow at your waist to maintain spine alignment

2. Back Sleeping with a Pillow Under Your Knees

Best for: Disc bulges, lumbar strain, upper back pain

Lying on your back distributes your weight evenly and helps maintain the natural curve of your spine. Adding a pillow under your knees flattens the lower back, reducing disc pressure and muscle strain.

Tips:

  • Avoid thick pillows under your head — keep your chin level
  • Choose a mattress that supports the natural curves of your spine
  • Place a rolled-up towel under your neck for extra cervical support if needed

3. Thin Folded Towel Under Your Lower Back

Best for: Herniated discs, Spinal Stenosis, Degenerative Disc Disease

Sleeping with a small Lumbar Support can take pressure off pinched nerves or inflamed discs. This is often recommended for patients with spinal stenosis or those recovering from lumbar flare-ups.

Tips:

  • If your folded towel is too thick and causing discomfort, only fold a portion of the towel, and leave the excess unfolded… might be easier to visualise rolling up a towel, but not rolling all of it up. – but instead of rolling it, fold it over on itself in the same way that you would if you were rolling it.
  • You may want to combine this with a pillow under the knees.

Positions to Avoid When in Pain

❌ Stomach Sleeping

This position forces your neck to twist for hours and causes the lower back to hyperextend — a nightmare if you have disc or facet joint problems.

If you absolutely must sleep on your stomach:

  • Use a very thin pillow or no pillow under your head
  • Place a pillow under your hips to flatten the lower back
